About shoulder pain
The shoulder is the most mobile joint in the body, which also makes it one of the easiest to irritate. Four rotator cuff muscles hold the ball of the upper arm in a shallow socket while the shoulder blade glides on the rib cage. Pain appears when a tendon is overloaded, the joint stiffens, or that coordination breaks down.
Common symptoms
- Pain when reaching overhead or behind the back
- Pain when lying on the affected side
- A catching or pinching feeling when lifting the arm
- Stiffness and reduced range of motion
- Weakness when lifting or carrying
Common causes
- Rotator cuff irritation or tears
- Pinching of tendons under the shoulder blade
- Frozen shoulder
- Arthritis
- Overuse from sport or work
- Pain referred from the neck
How we evaluate shoulder pain
We test shoulder range of motion and strength, watch how your shoulder blade moves, and use orthopedic tests to identify which structures are sensitive. The neck and upper back are examined because they often contribute. If a significant tear or other problem is suspected, we will refer you for imaging.
Care that may be considered
- Chiropractic care: Extremity adjustments for the shoulder, plus adjustments to the neck and upper back.
- Class IV laser therapy: Regularly used at the practice for shoulder pain.
- Shockwave therapy: May be considered for stubborn tendon problems.
- Neuromuscular re-education: Retraining the shoulder blade and rotator cuff muscles.
- Deep tissue massage: Soft-tissue work for tight shoulder muscles.
Recommendations depend on your exam. Not every option suits every patient, and results vary.

Frequently asked questions
Is my shoulder pain coming from my neck?
It can be. Pain that changes when you move your neck, or that travels below the elbow, often has a neck component.
Do I need an MRI?
Not usually at first. Imaging is recommended when the exam suggests a tear or when you are not improving.
Should I keep exercising?
Usually yes, with changes. Your provider will tell you which movements to keep and which to pause.
How many visits will I need?
It depends on the cause. You will get an estimate at your evaluation.
