About sciatica
The sciatic nerve is the largest nerve in the body. It forms from nerve roots in the low back and runs through the buttock and down the back of each leg. When one of those roots or the nerve itself is irritated, pain can travel along its path, usually on one side.
Common symptoms
- Burning, shooting or electric pain down the back of one leg
- Pain that is worse with sitting, coughing or sneezing
- Tingling or “pins and needles” in the leg or foot
- Numbness in part of the leg or foot
- A heavy or weak feeling in the leg
Common causes
- A bulging or herniated disc in the low back
- Narrowing around the nerve roots
- Tight muscles deep in the buttock
- Irritated joints of the low back or pelvis
- Prolonged sitting
How we evaluate sciatica
Your exam includes nerve tension tests, reflexes, strength and sensation in the legs, and an assessment of the low back, pelvis and hips. These help tell a disc problem from a joint or muscle problem. If the findings suggest you need imaging or a surgical opinion, we will refer you.
Care that may be considered
- Chiropractic care: Including flexion-distraction, a gentle technique often used for disc-related leg pain.
- Neuromuscular re-education: Core and hip retraining to take load off the low back.
- Deep tissue massage: Soft-tissue work for the hip and buttock muscles.
- Class IV laser therapy: Sometimes used along the painful area.
Recommendations depend on your exam. Not every option suits every patient, and results vary.

Frequently asked questions
Is sciatica always from a disc?
No. Discs are a common source, but joints, narrowing around the nerve roots and deep buttock muscles can produce similar pain.
Should I stretch through the pain?
Aggressive stretching can irritate the nerve further. Ask your provider which movements are appropriate for you.
How long does sciatica last?
Many episodes improve over several weeks. Pain that persists, returns or worsens should be evaluated.
Do you adjust the neck for sciatica?
The whole spine is evaluated. Care is directed to the areas your exam shows are involved.
